Football Camp begins on Thursday August 13th, 2026
All other Fall Sports Begin on Monday August 17th, 2026

Winter Sports Begin on Monday November 9th, 2026

Spring Sports Begin on Monday March 1, 2027

Sports Registration Info- 26-27 School Year- NEW!


What you need to know right now!  The process is changing!

1. To be eligible to participate in interscholastic athletics for the 2026-2027 school year, all student-athletes need a DIAA PPE (preparticipation physical exam form) completed by their physician or qualified healthcare provider after April 1, 2026.

2. 
DIAA has created a new PPE form.  This will be the only form accepted for the 2026-2027 school year. The new form will be posted on the DIAA and school athletic websites.  Please see the DIAA Medical Forms page (click link) for more information.

3. 
Appoquinimink School District will be using a new registration platform for athletics:  Rank One.  This will replace the SNAP Manage platform and is in collaboration with the DIAA.  Detailed instructions will be provided in May. No registrations or physicals will be accepted until mid May. 
 

Please feel free to reach out to your school’s athletic director with any questions once new instructions are provided.

General previously recorded Appo School District Athletic Information Video.  This includes information for the High Schools and the middle schools in our district. 

DIAA Sports Physical:
PHYSICALS FOR THE 2026-27 SCHOOL YEAR MUST BE DATED AFTER APRIL 1, 2026 
1. Make sure that you make a copy of all the forms and keep a copy for your records.
2. Make sure all signatures are in place before uploading into snap manage. if signatures are missing the forms are invalid. You will not be allowed to participate in any activities until all necessary forms are complete.
3. As long as the athlete has had a routine  physical exam (Well visit) within the last 12 months with their Primary Care Provider (PCP), YOU DO NOT NEED A NEW EXAM.  Their PCP can fill out the DIAA forms stating their last exam date was in the last 365 days and the clearance date has to be anytime after April 1st.  The exam date can be anytime within the last year.

WHERE CAN I GET A PHYSCIAL??

1. The Middletown School Based Health Center is open and scheduling appointments for students who will be participating in sports for 2025-2026 school year. The physicals are convenient and FREE to Middletown students.

For those who are members use the link below to complete the DIAA form and call us at (302-696-3140) or use schoology  messaging to Wellness Center Kristina Brandis to reach us for an appointment. 

 For those who are not members yet, please use the SBHC link given and the DIAA link to complete the form for your physical.

2. Med Express/Walk in urgent cares- about $30 (call ahead to make sure they do them and bring your blank 24-25 DIAA form)

 

3. Your Child's Pediatrician/PCP


 We no longer collect physicals in paper form. ALL sport registration & physical collection is done online. Please do not drop off any papers.
